Registration
Once your appointment is scheduled, we'll email all the new patient forms — so you can complete registration online from the comfort of home.
This speeds up check-in and minimizes waiting time. No need to print, fax, or email anything; simply fill the forms out completely and submit them online.
Companions
Patients can have a maximum of two (2) adults accompany them. Children are not permitted in the exam or ultrasound rooms.
Pets policy
To ensure the health and safety of our patients, Perinatal Associates and Testing Center (PATC) enforces a No-Pets policy. This includes emotional support, comfort, and therapy animals.
In accordance with the Americans with Disabilities Act (ADA), PATC permits working service dogs to accompany our patients. Service animals must be leashed or harnessed at all times, except when tethering would hinder their task. Service animals are not permitted in ultrasound exam rooms and must remain with a supervising adult in the lobby.
